1. ACCEPTANCE OF TERMS
By using our services, you confirm that you are at least 18 years of age and possess the legal authority to enter into this agreement. If you do not agree to these terms, you must immediately cease all use of our platform and services.
2. MEDICAL DISCLAIMER
THE SERVICES PROVIDED BY IV JUNKIE ARE NOT INTENDED TO DIAGNOSE, TREAT, CURE, OR PREVENT ANY DISEASE. THE CONTENT ON THIS WEBSITE IS FOR INFORMATIONAL PURPOSES ONLY AND DOES NOT CONSTITUTE MEDICAL ADVICE. ALWAYS SEEK THE ADVICE OF YOUR PHYSICIAN OR OTHER QUALIFIED HEALTH PROVIDER WITH ANY QUESTIONS YOU MAY HAVE REGARDING A MEDICAL CONDITION. YOU REPRESENT THAT YOU HAVE PROVIDED ACCURATE AND COMPLETE HEALTH INFORMATION AND THAT YOU HAVE CONSULTED WITH YOUR OWN PHYSICIAN AS NECESSARY BEFORE RECEIVING ANY TREATMENT.
3. ELIGIBILITY FOR SERVICES
Our concierge services are subject to medical screening. You agree to truthfully and completely disclose all relevant medical history, current medications, allergies, pregnancy status, and any other health information requested by our clinical staff. Our clinical staff reserves the right to refuse service to anyone based on medical history, current health status, or if we believe the treatment may pose a risk to the individual. Any misrepresentation or material omission of health information may result in termination of services without refund and shall release IV Junkie & Co. LLC from liability for any resulting adverse effects.
4. APPOINTMENTS AND CANCELLATIONS
Appointments must be scheduled in advance through our booking system. Cancellations made less than 24 hours before the scheduled appointment time may be subject to a cancellation fee. No-shows will be charged the full amount of the service.
5. FEES AND PAYMENT
You agree to pay all charges incurred by you or on your behalf through the service, at the prices in effect when such charges are incurred. We use secure third-party payment processors. IV Junkie & Co. LLC does not store your full credit card information.
6. CONCIERGE SERVICE LOCATION
We provide mobile services in the Jacksonville, Florida area and surrounding regions as specified on our site. You must provide a safe and clean environment for our practitioners. We reserve the right to leave any location if the environment is deemed unsafe or unprofessional.
7. USER CONDUCT
You agree not to use our website or services for any unlawful purpose. You shall not harass, threaten, or abuse our medical staff or employees. Violation of this provision will result in immediate termination of services without refund.
8. INTELLECTUAL PROPERTY
All content on this site, including text, graphics, logos, and images, is the property of IV Junkie & Co. LLC and is protected by copyright and trademark laws. Unauthorized use of any materials is strictly prohibited.
9. LIMITATION OF LIABILITY
TO THE MAXIMUM EXTENT PERMITTED BY LAW, IV JUNKIE & CO. LLC SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS OR REVENUES, WHETHER INCURRED DIRECTLY OR INDIRECTLY, ARISING FROM YOUR USE OF OUR SERVICES. IN NO EVENT SHALL IV JUNKIE'S TOTAL AGGREGATE LIABILITY ARISING OUT OF OR RELATED TO THESE TERMS OR YOUR USE OF THE SERVICES EXCEED THE TOTAL FEES ACTUALLY PAID BY YOU TO IV JUNKIE & CO. LLC DURING THE TWELVE (12) MONTHS IMMEDIATELY PRECEDING THE EVENT GIVING RISE TO THE CLAIM. YOU ACKNOWLEDGE THAT IV THERAPY INVOLVES INHERENT RISKS, INCLUDING BUT NOT LIMITED TO ALLERGIC REACTIONS, BRUISING, INFECTION, AND OTHER ADVERSE EFFECTS, AND IV JUNKIE SHALL NOT BE LIABLE FOR ANY SUCH OUTCOMES EXCEPT TO THE EXTENT CAUSED BY GROSS NEGLIGENCE OR WILLFUL MISCONDUCT.
10. INDEMNIFICATION
You agree to indemnify, defend, and hold harmless IV Junkie & Co. LLC and its officers, directors, employees, contractors, and agents from any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or related to: (a) your use of the services or; (b) your violation of these Terms; (c) your misrepresentation or omission of health information; (d) your failure to disclose any medical condition relevant to the services; or (e) your violation of any applicable law or regulation. IV Junkie & Co. LLC shall have the right to control the defense of any such claim at your expense.

ANY DISPUTE, CLAIM, OR CONTROVERSY ARISING OUT OF OR RELATING TO THESE TERMS OR YOUR USE OF THE SERVICES SHALL BE RESOLVED EXCLUSIVELY THROUGH BINDING INDIVIDUAL ARBITRATION ADMINISTERED BY THE AMERICAN ARBITRATION ASSOCIATION UNDER ITS CONSUMER ARBITRATION RULES, EXCEPT THAT EITHER PARTY MAY BRING AN INDIVIDUAL ACTION IN SMALL CLAIMS COURT. YOU AND IV JUNKIE & CO. LLC EACH WAIVE THE RIGHT TO A JURY TRIAL AND THE RIGHT TO PARTICIPATE IN A CLASS ACTION, CLASS ARBITRATION, OR OTHER REPRESENTATIVE PROCEEDING. Any arbitration or court proceeding shall take place in Duval County, Florida.
